PROGRAM STATEMENT

RETRAC Enterprises INC provides a caring and loving adult family home for residents who are unable to live independently within the community. We offer a capacity of 4 adults with developmental disabilities, emotional disturbances and/or mental illness in a home that is not accessible to individuals with mobility problems. We intentionally chose these client types due to their similarities in having disorders of thought, mood, perception or memory that can impair their judgment, behavior or capacity to recognize reality and meet the ordinary demands of life.

Our home consist of a 1100 square foot bi-level space with 4 bedrooms, 2 full bathrooms, 2 common area spaces, a dining area, a kitchen centered in the middle of the home and a large basement for entertainment and storage. Our home is located on the north side of Milwaukee and surrounded by access to public transportation, a community park and a 10-15 minute ride to downtown, museums, county zoo and summer festivals
Services at RETRAC will be provided by Amber Ellis, a Physician Assistant who has more than 12 years’ experience in the medical field, specifically psychiatry and by Ieaisha Tate, a medical assistant with more than 15 years’ experience in healthcare and hospitality. Other employees will meet the guidelines established by the licensing agency which include 15 hours of training related to the health, safety, welfare, resident rights and treatment appropriate to residents prior to or within 6 months of hire. This training will also include training in fire safety and first aid.

The mission and intent of RETRAC adult family home is to focus on the needs of the people we serve. Our goal is to make a significant difference in every life we encounter by utilizing the biopsychosocial model, also known as the mind and body connection to demonstrate healthy lifestyles, safety and overall well-being. This model introduced by a psychiatrist in 1977 has been used in different capacities to completely understand health and its issues.

Services offered

Staff present 24 hours a day, 7 days of week, 1 staff per 4 clients. *Increased client/staff ratio when deemed necessary through Individualized Treatment Plan (ITP)*

Biological

Ensure primary care health services are available, schedule follow up appointments and follow recommendations of primary care doctors for referrals, laboratory and medication management. Maintain secure health records for each adult for monthly review and continuation of care.

Psychological

Focus on management and stabilization of mental health conditions and disabilities through collaboration with mental health providers, therapists and other treatment team members. Once a month treatment plan review of mental health symptoms, changes, concerns and development of plans for improvement

Social

This service will include providing access and activities related to religion, economics, primary relationships, and cultural education with peer support groups to foster independence. A weekly schedule of activities and events posted to ensure a wide range of diverse opportunities.

Transportation

Owner, Amber Ellis, and house manager, Ieaisha Tate, will provide transportation Sunday through Saturday from 7am-5pm with at least 24 hour advance notice or request. Transportation after 5 pm requires 72 hour advanced notice. Emergency services (fire, police) will be contacted when needed for transport.

Other

Meals will be posted and offered three times a day with options for alternatives. Clients will have access to their individual cubbies for snacks. Storage and safe medication dispensing under the direction of treating provider. Laundry services weekly for clients unable to complete on their own. Access to internet, television, books, magazines, games and puzzles in common areas. Maintain organization of client property and belongings with inventory log and easy accessibility.
